Mail Security Content Filtering only working on internal mail

I created and enabled rules within the content managment portion of Mail Security for Exchange 2010. The rules work if I only select internal mail, they do nothing if I choose inbound or outbound. I read the manual and left the user(s) section blank  so that it would apply to all senders or recipients. What am I missing, it is like the mail is not going through the filter at all.
